I already know the answer to this question. Well, make that answers. The reason I am asking is that I would like to collect as many different translations as possible for a glossary entry.
I was just puzzling over which way to translate it in a document I was just working on. In my specific context, it referred to a 'form' that had been included at the end of a document. Various things came to mind, such as 'selection form', 'choice form', 'form', and 'option choice form'.
The real reason for this question, however, was just to see how many English translations people could come up with so I can stick them all in my glossary in CafeTran. For some reason, I am always puzzled when I discover that a word I thought would be in every dictionary is in none, yet.
