Eleven T-B

Elamite term or phrase: Eleven T-B
On a birth certificate, there is a small handwritten mention "Eleven T-B" or perhaps "FB" under the main certificate table on the left. I have never seen this before. There is one word struck out on the certificate but it is the word "Twenty" so not enough characters to relate to eleven. Additionally, the number 11 has been handwritten in the field marked 15 and just under that number in that field. I have no idea what either modification signifies neither does the bearer of the certificate.

Any ideas what this note under the table could mean?
