It is well documented that complications can arise from prescribing multiple medications. The likelihood of drug interactions and toxicity increase with the number of prescribed medications. To monitor, evaluate, and potentially adjust patients' medications can be challenging, in particular when multiple health care providers are involved. Multiple medication use is further complicated by the fact that patients often take non-prescription drugs in addition to prescribed medications. Multiple medication use may also increase the risk for medication errors. Therefore, monitoring medication use can help reduce multiple or inadequate prescriptions.
