Lampa rentgenowska obraca się wewn±trz kręgu nieruchomych detektorów wysyłaj±c podobnie jak to było jak w skanerach III generacji szerok± wachlarzow± wi±zkę promieniowania. Liczba detektorów zawiera się w przedziale od 600 do 5000. Zmniejszenie masy gantry i zmniejszenie liczby poł±czeń kablowych w czę¶ci obracaj±cej się umożliwia wykonanie kompletnego badania w czasie 1s. Systemy IV generacji s± generalnie droższe ze względu na duż± liczbę zastosowanych detektorów, jednak ich szybko¶ć umożliwia obrazowanie poruszaj±cych się organów dzięki zsynchronizowaniu ruchu obrotowego z sygnałami fizjologicznymi uzyskiwanymi np. z EKG. Tomografy tej generacji nazywa się rotate-fixed ze względu na obrotowy ruch lampy wokół nieruchomego pier¶cienia detektorów.
