high potential learning event
veľmi pravdepodobná udalosť

Explanation:

udalosť, pri ktorej mohlo dôjsť k smrteľnému zraneniu, so vzdelávacou hodnotou

Explanation:
Ja by som to riešil opisne, aby sa nič v preklade nestratilo.

Report 3 has been changed from “Significant Incidents” to “High Potential Events” to capture more detailed information situations which had the potential to cause fatalities.
A high potential event is defined as any incident or near miss that could have realistically resulted in one or more fatalities. The intent of Report 3 is to maximise learning from these events... Organisations are requested to limit the number of high potential event reports to those which have high learning value for the industry...

High Potential Events are defined to be any incident or near miss that could, in other circumstances, have realistically resulted in one or more fatalities.

Any event (incident or near miss) that could have resulted in one or more fatalities but had the potential to do so in other circumstances.

We would like to thank the vessel which reported this High Potential Learning Event... we collectively know what needs to be done in order to learn from this event to ensure that something like this never happens again; and so we may all return safely to our families.

Prípadne by som použil "udalosť s potenciálom pre vznik smrteľného zranenia so vzdelávacou hodnotou".

high potential learning event
Nebezpečná udalosť s vysokým poučným potenciálom

Explanation:
Ja by som to takto formuloval z nasledovných dôvodov:

Došlo k nebezpečnej udalosti, ktorá očividne nemala následky, ale dotknutá strana uvádzka: High Potential Learning event - teda udalosť má poučný potenciál do budúcnosti. (pre dotknutú stranu či iné osoby).
