Creo que quieren decir que las relaciones tienes que ser "abiertas" o "transparentes", en el sentido de que tienen que ser "claras y evidentes", "que se comprenden sin ambigüedad", de manera formal.

De forma "oficial", tienen que ser muy claras. Ahora, si el paciente se entera o no se entera de lo que es un "trastorno catarral del tracto respiratorio", o un "costipado", eso ya es otro cantar.

Y ya lo decía mi abuela: "Un costipao mal curao: un tísico confirmao"... ;-)
