MI (Middle Initial, Inicial)

I'd go for the following translation:

NOMBRE, Apellido, de pila, inicial

I imagine this is in a US context, because it would be lackint the possibility of the two last names in Spain ans certain countries in South America, as well as that Inicial, which is basically unknown in Spain.
