English term or phrase: physician associate / physician
Dear native English speakers!

I am translating an extract from a text written in the early 18th century, where a man who studied medicine was first appointed as a doctor's assistance, and then, after a period of time, he became a doctor.

I'm looking for English terms that were most typical for the 18th century and may look somewhat old-fashioned today.

Being no native English speaker, I can't be sure which of the following terms are the best to meet this requirement. Please advice!

physician associate / physician
physician assistance / physician
assistant physician / physician
doctor's assistance / doctor
doctor's associate / doctor
auxiliary doctor / doctor
medical assistant / medic(?)

Should anybody come up with any other terms that would look somewhat old-fashioned, I'll appreciate that very much!
