English term or phrase:"loop off the pin" corrective and preventive actions
The review included (name of drug) complaints to determine the success of the previous "loop off the pin" corrective and preventive actions. No negative (name of drug) trends were noted relevant to "loop off the pin".
I think that it's an idiom, but I've failed to find it. Can anybody explain it? Please!
True, but that would be a significant loss. If you think the target audience can handle a bit of English, you could include it as a foreign expression in English, possibly with a footnote by way of explanation.
Then I suspect that 'loop off the pin' is a name that has been given to a particular problem identified in a certain context, which perhaps concerns a situation in which a loop of something is not around a pin as it should be.
Not an idiom (i.e. figurative usage) that I've ever encountered. Is the document source NES, and are the quotation marks present in the source text?

A control system that does not have a feedback loop and thus is not self-correcting. www.thefreedictionary.com
An open-loop controller is often used in simple processes because of its simplicity and low-cost, especially in systems where feedback is not critical. A typical example would be a conventional washing machine, for which the length of machine wash time is entirely dependent on the judgement and estimation of the human operator. Generally, to obtain a more accurate or more adaptive control, it is necessary to feed the output of the system back to the inputs of the controller. This type of system is called a closed-loop system.
A closed-loop process is one in which:a problem is identified, the root cause of the problem is determined, a corrective action plan is identified to eliminate the cause, the corrective action is implemented, and the effectiveness of the corrective action is verified http://188.8.131.52/search?q=cache:fLK9huIqefEJ:www.hq.nas...
Open Loop (Manual) or controlled vs. Closed Loop (Automatic)
Comparing open-loop control with closed-loop control the following differences are seen:
shows a closed-loop action (closed control loop);
can counteract against disturbances (negative feedback);
can become unstable, i.e. the controlled variable does not fade away, but grows (theoretically) to an infinite value.
shows an open-loop action (controlled chain);
can only counteract against disturbances, for which it has been designed; other disturbances cannot be removed;
cannot become unstable - as long as the controlled object is stable.
Summarising these properties we can define:
Systems in which the output quantity has no effect upon the process input quantity are called open-loop control systems.
Systems in which the output has an effect upon the process input quantity in such a manner as to maintain the desired output value are called closed-loop control systems.
Therefore, the "loop off the pin" would (maybe) be a human controlled system for managing corrective and preventive actions - an open-loop system.
