That is probably the most literal rendition, but not necessarily the best. Other possible phrasings:

عدد كافٍ من قسـيمات الإجابات الدولية، مالا يقل عن خمس
ما لا يقلُّ عن خمسٍ من قسـيمات الإجابات الدولية

The last one is my favorite.

Coupons are also called كوبونات

Knowing what this application form is about and what coupons are indicated may help in rendering this differently, perhaps more effectively. The element I am somewhat concerned about is the adjective “international,” which could mean a number of things.
