I suggest that the asker go with the term suggested by Fuad. That is virtually identical to the form used at some health centers, cancer institutes, and hospitals here in southern California.

On the other hand, if the asker intended to refer to a "living will" in the context of a changeable legal document that applies outside a medical context, perhaps there is another and different equivalent expression.
