"Authorization to Release information" is the title of a form that patients are required to sign so that their hospital can "release" their medical information to insurance companies, other hospitals or third parties.

I have seen this expression translated in a variety of ways: NASHR, ITLAQ, even 'ITQ (Just don't tell anyone I said that).

I am not happy with NASHR, because it sounds like releasing the information for publication, and I am not happy with ITLAQ, because it sounds like launching the information in a rocket. Needless to say, 'ITQ is entirely inappropriate. What would you suggest? I am thinking of TOWSEEL AL-MA'LOOMAT, but it does not give the sense of "releasing" essentially confidential information. Would IFRAJ be acceptable? What about IFSAH? I mean both إفساح and إفصاح

I am open to all ideas.

Explanation:
I’ve seen the words ifraaj and ‘itq in many documents, I’ve also seen other terms like:
- ‘i’efaa min mas’oleyat إعفاء من مسؤولية كذا وكذا..
- ibra’a themma min إبراء ذمة من..

if it comes to me I would use: ifshaa’ إفشاء from afsha assalam أفشى السلام

Other possibilities:
albawhh bi.. البوح بـ
Tanaqul تناقل
Tadawol تداول
Modawarat al.. مداورة الـ
Ifsah or ifssah don’t fit.

Some military and commercial references I checked give these examples:

o nasher نشر

o iTlaaq اطلاق

o taHriir تحرير

An approximate (albeit-longer) suitable term might be:

Tahriir (or nashir) al-ma3lumaat al-Tibbiiya lilmasmooh lahum faqat

تحرير (نشر) المعلومات الطبية للمسموح لهم فقط

or the variants of

"yesmaH al-tahriir..

or

"mamnu3 al-tahriir... lilghair al-masmooH lahum"

Since the term is the title of a form, perhaps "Ijaazat nashir ma3lumaat tibbiiya" might be suitable.

FWIW, a similar medical form I saw in Amman, Jordan said "RahsSa towzi3..." with boxes for entering at least six authenticating signatures before one page ever left the department.

Explanation:
My suggestion would be to stay away from the direct rendering of 'release'.

I suggest you use
"As-Samaah Bitawfeer Al-Ma'loomat Liman Yahimuhu Al-Amr". This states that you make the information available to whom it may concern and it implies releasing of information to those concerned.
