I think that medically مدرك is the right meaning and it came in the text to confirm the patient's consciousness of the surrounding factors. I don't think that متوافق/متكيف is a right translation in this context, it doesnot give the medical required meaning.

However, medically, "conscious" is the state of being "awake, or not "unconscious" which is حالة الغيبوبة" , whereas "oriented" means the state of mind that implies the knowledge of the surrounding factors, be it time or location or persons.., which is translated in arabic as "مدرك". It cannot be مدرك لما حوله for two reasons: First, conscious is مدرك لما حوله or واعي if you like. Second, in this context it means متوافق/متكيف so I agree with Taghreed and totally disagree with the other colleagues with all do respect.
