At the termination of the employment contract, the employee must return to the society where he worked all the goods that he used and which belong to the society according to the employee departure clearance form
I think all the suggestions so far correspond to 'employee departure form' — but I'm not sure this tells the whole story!
If I understand the explained context correctly, this 'clearance form' is going to (no doubt amongst other things!) list all the items the employee must return before they are 'cleared for departure' — and if this is indeed the case, then I think it's important that the notion of 'clearance' be rendered in some way in the translation.
I was toying with the idea of 'décharge' — a term that crops up quite often here as a 'release'; but 'release' and 'clearance' do often have very similar meanings in these sorts of context.
