Patent ductus arteriosus, or PDA, is a malformation of the heart caused by failure of normal closure of the ductus arteriosus, a direct connection between the aorta and the pulmonary artery which is present and functional during fetal life but which is unnecessary once air-breathing begins. Normally the ductus closes shortly after birth; this process of closure by muscular contraction is reversible for a short time - it can be reopened under hormonal control (prostaglandins). However, within a few weeks it is permanently closed.

Def de persistance du canal artériel:

La canal artériel relie l'artère pulmonaire à l'aorte descendante. Il se ferme normalement dans les heures qui suivent la naissance. Sa persistance anormale est plus fréquente chez le prématuré que chez l'enfant à terme.

Les symptômes sont ceux des shunts gauche-droite : infections respiratoires, hypotrophie staturo-pondérale, souffle cardiaque continu maximum au 2° espace inter-costal gauche. L'évolution vers l'hypertension artérielle pulmonaire irréversible est rapide si le shunt est important. Le risque d'endocardite impose la prévention habituelle. L'ECG est normal si le shunt est faible ou montre une HVG. Sur la radiographie thoracique, la vascularisation pulmonaire est augmentée et le volume cardiaque également.
