La plupart des essais de médicaments nouveaux sont faits en comparaison avec un médicament de référence connu dont on connaît l’efficacité (comparateur). On parle de simple aveugle quand seul le patient ne sait pas dans quel groupe (bras) il se trouve durant l’essai (ce dernier y étant placé après tirage au sort : randomisation) et de double aveugle (double insu) quand ni le patient ni le médecin ne connaissent le bras thérapeutique. Les essais les plus fiables sont pratiqués en double aveugle mais ils peuvent être impossibles en raison de modes d’administration différents des produits (perfusion et comprimés). Dans ce cas, le double aveugle peut porter sur l’interprétation des résultats en imagerie (IRM).
