Ich habe hier nur den affidavit of identity = "eidesstattliche Erklärung zur Person" gefunden.

Affidavit of identification of X (applicant) in respect of service of application for order dissolving marriage or civil union.

Aber eigentlich müsste das ja die gleiche Erklärung sein, oder?

Identitätsnachweis geht zu weit, wenigstens steht dort nichts davon, dass Ausweispapiere vorgelegt wurden.

ich glaube auch, dass affidavit of identity und affidavit of identification gleichermaßen eidesstattliche Identitätsnachweise bezeichnen.
Sieh dir dazu doch einmal die beiden Formulare an:

Es handelt sich in beiden Fällen um ein affidavit of identification und beide sind eidestattliche Erklärungen zu einer Person.

Auf http://www.wisegeek.com/what-is-an-affidavit-of-identity.htm wird außerdem erklärt, dass der Identitätsnachweis im Falle eines affidavit of identity in manchen Fällen, neben der eidesstaatlichen Erklärung, auch durch den Personalausweises erfolgen kann / muss. Daher finde ich auch Identitätsnachweis als Übersetzung akzeptabel.
