I think it means that if the Applicant, or solicitor) has not cancelled the requirement, when you receive the notice, yhou must send an Affidavit to the court, with all the details of your property and income, and a fee of $10. You must also send a copy of this to the solicitor.

Unless (they say you need not to, i.e. no requirement), you have to (and this is the requirement:) send or deliver an Affidavit to the Courts and at the same time to send a copy to the solicitor. And that either (within 22 days after receiving the notice in addition to sending a form of appearance) or (within 14 days after receiving the notice).

So it is more simple: Unless A, then B and at the same time C.
