unfortunately there is no additional context
available. It is just in notes on what the company
provides the client/public. As far as I know, there is no such thing in German law, but it needs to be translated somehow.

As far as I know, a "disclosure statement" is a sort of "Offenlegung" or "Bekanntgabe" or "Bekanntmachung" or "Veröffentlichung". At least these are translations suggested by Zahn, Romain and Hamblock/Wessels.
