The CI will exercise his/her best efforts to screen and qualify at a rate of eight (8) to ten (10) cases per month for participation in this study. The determination of "Qualified Patients" will be based on the inclusion and exclusion criteria in the above referenced Protocol (XXX Protocol …-001).
(b) XXX shall pay the CI one thousand dollars US ($1,000US) as follows:
a. Half (50%) of the amount per patient, five hundred dollars US ($500 US) will be paid to the CI after:
i. Each procedure and the return of patient data forms to include BOTH the pre-operative and day one post-operative follow-up forms and radiographic data (MRI and AP and lateral spine films).
b. The remaining half, (50%) five hundred dollars US ($500 US) will be paid after:
i. The successful completion of BOTH the one (1) and three (3) month follow-up to include submission of the patient data forms and radiographic data (CT and AP and lateral spine films).
c. Payments will be made within 30 calendar days after receipt by XXX of invoice and completed data forms to the following account:
XXX shall pay an additional EUR 400 per month for a total of four (4) months for support staff to work with the PI in an effort to expedite the **EC paper work** and the enrollment and maintain the necessary records.
