/ insurance/medical records' request
English term or phrase: Magnum Event L
This is just a bit puzzling to me. A letter to an insurance company, which had requested medical records from a clinic, lists the following line in its subject heading:

Magnum Event L ,
followed by Social Security # and Date of Birth.

Is there a first name 'Event'? Or is this relating to something else? i am aware of the name Magnum as being a last name.

Thanks for your input!
