1. If you do not completely translate your form into the target country languages, you will reach only a small proportion of the Internet population. Those you do reach may be insulted that you didn't take the trouble to translate your form into their language.
2. You can use applications like AltaVista's SYSTRAN Babelfish but the results are quite bad and might draw a few laughs from your target audience, but not many information requests.

The above came out poorly sorted. I have straightened out the order a little but it is still completely unusable. Here is what it looked like straight from Babelfish:

The result is quite funny, take for example the German word "Pfosten", it means "post" but not "mail", it means "post" like "fence post". The German word for "mail" is "Post". The German word for "Email" though is "E-Mail".
That should clear up everything. Anything that is worth doing is worth doing well.
