Kérem, értesítse a szakácsot vagy főszakácsot!

Explanation:
Well, "notify" is a little bit too formal for a restaurant setting, so if you just wanted to say "call" instead, then here's the sentence:

Kérem, hívja a szakácsot vagy főszakácsot!

neutral  xxxivw: "notify" here means "let them know" (that I have these allergies), so "call" is definitely not the right choice of word.

agree  Attila Piróth: OR: Kérem szóljon a szakácsnak (vagy a főszakácsnak). You might as well omit the second, it is quite redundant.
5 hrs

disagree  Eva Blanar: instead of "főszakács", I'd use "konyhafőnök" or "séf".

Kérem, tudassa a szakéccsal vagy főszakáccsal.

Explanation:
Again, I suspect this belongs to the full text you need translated but posted in bits and pieces here.
So, you wish to notify the cook / chef of your allergies to make sure you won't be served any items that might be dangerous to you.
kérem - please
tudassa - notify
cook - szakács
főszakács (or séf ~~séffel) - chef

Kérem szóljon a szakácsnak (vagy a főszakácsnak).

Explanation:
Szóljon is a versatile word: it can mean either `Tell the cook what I have just told you’ or `Call the cook’. I would also suggest omitting the second part, (especially if the sentence means the 1st above); `szakács’ (cook) is a general-enough word to include the chef as well, therefore using both is unnecessary. The verbs in the 3 other solutions, ie `tudassa’ `értesítse’ and `tájékoztassa’ are rather formal; `szóljon’ is completely down-to-earth.

disagree  xxxivw: "notify the cook" is not the same as "tell the cook" and especially not "call the cook." "notify him" means "let him know" - which in Hungarian is "tudassa vele." If it was (but it wasn't) "tell the cook," your translation would be near correct.

neutral  Eva Blanar: Actually, "szóljon" is quite versatile, but that "főszakács"...(konyhafőnök / séf)

-> főszakács/konyhafőnök / séf are synonyms but I suggest one should omit them.

agree  Csaba Ban: egyetértek, hátha automatikusan odaítélődik a pont, ha már a kérdező nem veszi a fáradtságot...

Kérem szóljon a szakácsnak, vagy a főpincérnek!

Explanation:
A "cook" egyértelműen "szakács", a "chef" lehet a főszakács (séf) is, de szerintem a "főpincér" jobban passzol ide.
