"A cretino!" or "Ma chi ti ha dato la patente?!"

Explanation:
This really depends on the context. Depending on the intonation, this can be a provocative, mild or very offensive statement! My first suggestion is to be read in Roman dialect (given the slang yiddish, I thought I could use some "poetic" license!). The second one (literally "Who gave you your driver's license?!") comes, I think, from a movie (w. Franco Franchi and Ciccio Ingrassia) and is generally accepted to mean "How dumb can you be?!"

Buon lavoro,

Floriana

agree  CLS Lexi-tech: come la corrispondente espressione italiana che a forza di usarla ha perso un po' di referenzialità.

razza di deficiente!

Explanation:
Schmuck = a clumsy or stupid person; an oaf.
[Yiddish shmok, penis, fool, probably from Polish smok, serpent, tail.]
