| afp定量精密/clia FYI Explanation: Clinical Laboratory Improvement Amendments (CLIA) 臨床検査改善修正法案（米国におけるすべての臨床検査行為の調査と規制のための米国の法律．医療財政局（HCFA）の後援で人的事項および手法を定めたもの）． 1988年の臨床検査改善修正案（CLIA'88）は臨床検査，特に診療所での検査やPap塗抹検査の判定の内容に対する一般の人々の関心に対応して議会を通過した．この法律によって，診療所の検査室を含めた15万の米国の臨床検査所がすべて同じ規制を受けることになった．臨床検査とは，疾病の診断・予防・治療あるいは健康診断のための情報を得るために人体由来の材料を検査する業務をいう．臨床検査担当者や方法の基準は検査の複雑性と患者に対する危険性によっている．法規にはCLIA登録や施行および調査の方法や費用，さらには検査所が基準に合わなくなったときの罰則も定められている．CLIAの規則では，検査の内容を，きわめて容易，中等度，高度の3つに分類している．医師が行う鏡検の亜分類は中等度のレベルとなっている．中等度ないし高度の検査では，別の検査所が定期的に既知の構成分のテスト用試料を送って行う能力試験プログラムに参加しなければならない．CLIA法の負担や強化に対し，特に診療所で検査を行っている個人医師から反対が起きている．法律に反対する人は，CLIAの規則は患者の医療の改善にあったとしてもほとんど明らかな効果がないにもかかわらず，診療所での検査の規制は医師が患者のニーズに役立つのを妨げていると主張している．患者や第三者の支払い側はCLIA規制に合わせるための臨床検査代の増加を負担しなくてはならなくなっている．さらに，およそ1/3の医師はCLIAの結果，一部ないしすべての診療所での検査を中止している．これにより，患者と医師の双方により多くの不便さと費用がかかっている．特に小児やお金のない人や高齢者にとって，何回も受診したり，モニターのスケジュールに従うことは難しく，このため患者の治療全体の質の低下がみられている．また医師への検査結果の遅れは，患者のコンプライアンスを低下させ，診断の遅れや間違いを生み，予測に基づく治療となり，不必要な経費がかかり，ときには不要な入院となることもある． ステッドマン |
