スペイン語かラテン系オリジン？（スペイン語の文献に多くでてきますね）日本語の定訳は見つかりませんでしたが、英語の文献では使用されているようです。subはよく下方を意味するので肺の下方に異常が認められる肺繊維症なのではないでしょうか？CTでsubyacentに異常が認められないといった文脈があるので、肺繊維症のCT検査関連の文書で検索してみてはいかがでしょうか？私は今、忙しくて調べることができませんが、少しでもヒントになれば幸いです。

「下方」ではなく「下肺（野）」だと思います。
下記URLより、肺繊維症で下肺生検を行うこと、また、膠原病肺に「下肺野繊維症」の名称が使用されていますので、「下肺（野）」が妥当ではないかと思います。
