Explanation:
Seemingly, your text is not purely medical:) It is written for pupils, say ordinary people, ... at least not high-pro medicians.

Recall that among the most dangerous symptoms of rheumatism one can find, e.g.

One doctor said : the rheumatism licks joints and bites the heart! This is related to PERMANENT UNCOMFORTABLE SITUATION OF THE WHOLE BODY AND EXPECIALLY OF THE WHOLE c a r d i o v a s c u l a r s y s t e m.

Among the heavy consequences of the rheumatic state medicians indicate to a series of diseases which can be in general called as the rheumatic heart disease.

Below I simpy cite my very beloved Britannica (:=):

Rheumatic heart disease.
Rheumatic heart disease results from inflammation of the heart lining, heart muscle, and pericardium occurring in the course of acute rheumatic fever, an infection with Streptococcus pyogenes organisms. The disease includes those later developments that persist after the acute process has subsided and may result in damage to a valve, which may in turn lead to heart failure.

Rheumatic fever is poorly understood. The disease process occurs days or weeks following the initial streptococcal infection. Later infections may bring about recurrences of the rheumatic fever that damage the heart. Immunologic processes (reactions to a foreign protein) are thought to be responsible for the response that damages the heart and particularly the heart valves. Rapid and effective treatment or prevention of streptococcal infections stops the acute process. Many other factors of a geographic, economic, and climatic nature influence the incidence of rheumatic fever but are not the primary causes. Rheumatic fever is becoming less common in the second half of the 20th century, and with better control of streptococcal infections, there is an indication of a sharp decline in rheumatic heart disease.

Well, the most popular (and rather stable) term for this desease is РЕВМОКАРДИТ, but such diseases as перикардит might also occur.

Recall also that ABBYY's Lingvo 6.5 gives us:
rheumatic heart disease = ревмокардит (муж.; мед.)

Я думаю, что для данного вопроса ответ р е в м о к а р д и т самый подходящий. Но в современной медицинской литературе ревмокардит проходит как заболевание сердечно-сосудистой системы в с л е д с т в и е системного (ревматоидного) заболевания или как последствие плохо диагностированного и леченного заражения стрептококками.

Just an addition:
If the context inplies a permanent cardic defect, then the exact translation would be "ревматический порок сердца".

Explanation:
