Throughout the years, practices have evolved from the needle towards the cannula, which appears more secure, less painful, and causes less bruising.
Technique: a pre-hole of a larger diameter than the cannula enables the procedure to be more comfortable, especially when inserting the cannula. Cannulas are sometimes delivered as a kit containing the needle necessary to make the pre-hole, but it is of the same calibre as the micro-cannula.
Rule: the deeper the injection area, the more obvious the choice of a blunt micro-cannula is. Indeed, during a deep injection, the tip of the needle is blind and may pose a risk to nerves or blood vessels. On the opposite, a more superficial injection will be easier to perform with a needle.
Another safety rule: near where neurovascular bundles emerge, a cannula is preferable. We immediately think of the supraorbital ridge, the suborbital and the chin area as well as the top of the nasolabial folds on the sides of the nose, near the nostrils.
With a needle, there is always a risk of pricking a nerve or a blood vessel, even piercing through it. Even worse would be to inject into the blood vessel, causing a localized necrosis. Fortunately these incidents are very rare, even with needles. They are even rarer with cannulas, and even less likely if one takes the precautionary measure of systematically checking by aspiration before the injection.
Advantages of the cannula: its flexibility, its safety thanks to its blunt tip and a minimized trauma for tissues due to its blunt tip.
