English term or phrase: C/D ratio, optic discs, flatness
the first two are ophthalmological terms, the third is a term that describes the percussion of the chest. Also, does anyone know how to distinguish between 'domiciliary care' and 'home care' in Spanish--I only know to use the same adjective for both--domiciliario, but in my document I have them listed as different things.

optic discs=discos ópticos

flatness =same as dullness = matidez
(segun F. Navarro: "Para referirse al sonito mate que se percibe en la persucion, los medicos espanoles han recurrido tradicionalmente al galicismo "matidez" (del frances matite) = hepatic dullnes (matidez hepatica; incomplete dullness o partial dullness (submatidez); shifting dullness (matidez desplazable).

Explanation:
That's tough... but perhaps you can translate one as "domiciliario" and the other "a domicilio" ;o)
