I'd appreciate your help in finding a proper translation for this term.

This is not the actual text I'm translating, simply a text I found on-line that explains what nurse care managers do:

The typical duties of our ***nurse care managers*** include the following:

* Review clinical information and conduct initial assessments for new referrals.
* Address the needs of high-risk patients. (For example, follow hospitalized patients, assist with discharge planning and conduct follow-up contacts post-discharge.)
* Assist new dialysis patients with any medical, educational or logistical needs. Review their medications, nutritional status and lab results.
* Assist existing patients with any clinical outliers, new or existing patient concerns, new or changed medications.
* Work with the patient‚’s health care team to facilitate continuity of care. It is our aim not to add a layer of bureaucracy, but to collaborate with and maximize the strengths of the existing system of care.
* Work with the patient’s health plan to establish authorizations for treatments, procedures, hospitalizations, DME and home health care. Review cases with health plan’s medical director as necessary.

Acerca del género, copio la entrada del DPD para 'jefe':

jefe. 1. ‘Persona que manda sobre otras’. Por su terminación, es común en cuanto al género (el/la jefe; → género2, 1a y 3c): «Campos fue en aquella ocasión la jefe de expedición» (Tiempo [Col.] 11.1.87); pero cuando el jefe es una mujer ha sido siempre más frecuente el uso del femenino específico jefa, registrado en el diccionario académico desde 1837: «Habían decidido promoverla a jefa de un área que no existía hasta entonces» (Martínez Vuelo [Arg. 2002]). Sobre su escritura con mayúscula o minúscula inicial, → mayúsculas, 4.31 y 6.9.

2. Cuando jefe se usa en aposición a otro sustantivo, lo normal es que concuerde en número con este (→ plural, 2.4): «Eso lo repiten mucho los redactores jefes» (Carrión Danubio [Esp. 1995]). Si el referente es una mujer, puede usarse indistintamente la forma común jefe o el femenino específico jefa: redactora jefe o redactora jefa; no es admisible la expresión Marca de incorrección.redactor en jefe, calco censurable del inglés editor in chief.
