English term or phrase: Superbill
Context: Participating dentists have XX claim forms. If the dentist does not participate, the employee may bring one to the dentist. Superbills, however, are not acceptable.

Any idea what to call these "superbills" in Spanish? Apparently, they are some kind of bill submitted by the dentist to the insurance company. I can't very well call them Superfacturas now, can I?

Medicare will not accept *******non-standard claims. Non-standard claims (i.e. "Superbills")********can be defined as claims with extraneous attachments that are submitted by providers of service and (or) suppliers in lieu of entering the required information in the designated blocks on the HCFA-1500 (12-90) claim form. Claim attachments will be accepted only for information and evidence that cannot be readily entered in designated blocks of the standard claim form (i.e. medical records, certificates of medical necessity, other certifications or claim attachments required by law, regulations, or HCFA instructions).

This is a complex Medicare concept taht I can't quite figure out, but probably it's to do with the kind of claims that are payable by medicare. I suggest that you translate it as 'recibos no estandar' and footnote it.

En México a esta clase de formularios se les conoce como "Formas continuas". Para este caso específico, sugiero lo siguiente:
"Sin embargo, no se aceptarán los recibos de honorarios ni las facturas que se compran en los establecimientos que venden formas continuas".
