If this is not a person you know well use "pone" instead of "pones".

Me das grima


Explanation:
The other suggestions are fine, too, but don't give the full effect of the original sentence.
"Me pones la piel de gallina" or "Me das escalofríos" are a bit "light"... they would be better translations for something like "You give me the creeps".
Someone who "makes your skin crawl" is usually someone truely revolting... who makes your stomach turn. I think "Me das grima" more closely conveys that feeling.
Anyway, that's my $0.02-worth... ;-)
