The Social Security Administration sends people a "disability starter kit" when they make an appointment to apply for disability benefits. The kit contains a factsheet, a worksheet and a list of the items they will need. In English they are called "Adult Disability Starter Kit" and "Child Disability Starter Kit"; how can I say this in Spanish?
