Vill du gå ut med mig?

Explanation:
Vill du = Do you want to/would you like to
gå = go
ut = out
med = with
mig = me

"Will you" technically means "kommer du att", but in Swedish it wouldn't sound right to say it that way, it would sound more like " Are you going to go out with me?)

Skulle du vilja gå ut med mig?

Explanation:
Depending on the delicacy of the situation one could perhaps rather use
'Skulle du vilja gå ut med mig' which really is a translation of 'Would you like to go out with me?'

  -> Geriatric! Just well educated and polite and maybe more tactical!
