Is it likely that a doctor would have a Degree in all four specialisms?

Or should I translated 'Specialist in .....' for each?

This is how we would say it in the US. "Diploma" here is almost universally reserved for the fundamental milestones of education (high school diploma, university diploma [BA/BS]). Shorter courses that do not result in a BA/BS, master's degree, doctoral degree or equivalent (e.g. MD/medicine, JD/law) are usually called certifications, certificate programs or the like.
