See the additional context in the discussion box. This is very bitty (which is not Chris's fault), but the general idea of "accompagnement" seems to be "career development".

"Dispositif" is something that's made available, so I think it implies some kind of plan or programme. And a schéma is a chart, diagram, or other form of visual representation.

This may not be the perfect translation, depending on exactly what form this takes, but I think it's the right idea.
