This expression appears in a operation note for a patient undergoing sub-total colectomy. My guess is that it is something to do with remuneration according to French hospital agreements. But that's just a guess. The entire text is, "1 - HL PR COLLET BLOC DI (name of surgeon). Majoration transitoire de chirurgie. Majoration forfaits modulables pour chirurgien secteur 1 ou 2 optionnel, pour actes admettant J4 1 - HL SAR2 MHL/USN (name of another surgeon) HHCA002 0 Colostomie cutanee, par laparotomie."

I understand that this now allows surgeons to increase some of the fees which are established at a certain scale by the Social Security, depending on the procedure involved, as it may deviate from the "norm"...do you think?
