This appears on a 'requistion a personne' for a medical examination:

à l'effet de procéder aux actes ci-après
Effectuer un examen médical sur [name of person] , née le [D.O.B] au [place name], déclare être victime de violences conjugales.
Procéder à tous prélèvements utiles:

médicaux,
toxico-urinaire,
prélèvements de toutes traces laissées par le ou les auteurs (en vue d'analyse A.D.N)

I am not sure if 'Toxico-urinary' is the correct translation for this.
