Explanation:
The French expression "par nos soins" refers to the fact that "we have taken care of doing it". In other words, "this is something that we have done". In a translation, it would come out as follows:

"These facts have been sanctionned (approved, ratified) in the minutes (report, record) which we have countersigned."
