I can't seem to find any examples of an English equivalent of this. "Included rate table" comes up in Google quite a bit but the "included" seems to mean "included in the document".
I don't think we really have enough context to answer this. It may not be your fault, but for example we don't know what the "montants" are, or how they're being "traités".
I would expect there to be clues elsewhere in the document about what "taux inclus" are. But if not, then it has to be a literal translation, " table of average included rates".
