Bus/Financial - Insurance / life insurance policy
French term or phrase: bulletin de souscription
Following my previous question, this appears on the first page of the document, just below the heading "Contrat d'assurance sur la vie en devises et à capital variable". I've been looking at the different uses of "souscription" and am a bit wary of this word; here it would appear to be an application form (in fact, a form in which to insert all the details of the insured follows) but I'd like to be sure if "application form" is ok in the context.

MT again.

agree  Marc Glinert: I would actually go with policy application form - we used to say policy proposal form in the industry, but that sounds a bit dated now
