Literally, "where having been present and spoken to [the person on whom the process has been served]..."

In your document, it continues with "...as mentioned in the annexed [or "attached"] notice...."

As an alternative to rendering it literally, you could always simply paraphrase: "The addressee was seen and spoken to personally by the undersigned..."
