This refers to a medical condition. The context is an article concerning transplants.

Explanation: As noted above, the phrase "heart failure" can stand in for "cardiac insufficiency"; however, the latter is the more general term, embracing functional difficulties as well as fatal conditions.
The French actually refers to two medical conditions (one total, the other limited to the right side of the heart), using the semicolon and the word "de" as dividing indicators. (Obviously, heart failure or cardiac insufficiency cannot be both total and limited to the right side!)
