A 42-year-old woman with a basal cell carcinoma (BCC) on the left upper lip, located just at the facial-lip edge (Fig. 3). We excised the tumor with a 0.5 cm margin of normal skin and a triangular defect was left. An elliptic or crescentic defect was created above the triangle (Fig. 4A) in order to close it without tension (Fig. 4B). Suturing was begun and, as the long lateral skin edge was brought closer to the shorter medial skin edge, the cheek flap automatically advance and the vertex of the triangle was sutured under the nasal wing, closing the defect (Fig. 4C).
