I'm not quite sure what this means - I think the problem is the word préférentielle - I did wonder whether it was being used in the sense of predisposing or underlying. here's the context:
Les données échographiques transthoraciques (ETT) révélaient une altération sévère de la fonction systolique ventriculaire gauche homogène,** sans atteinte segmentaire préférentielle **
This is the context of a patient admitted to the ICU without any previous cardiac problems, probably as a result of a drug-induced ADR.

Explanation:
I understand it to mean that there is indeed damage but the damage is NOT to any specific segment. The "sans" is important to the meaning, like "sans préférence". I think "underlying" or "predisposing" would be an over-translation.

I agree that the above "sans atteinte préferentielle" suggests a more generalised effect. I would therefore lean towards this sense.
When looking for "atteinte préférentielle" I found this reference to crones disease, which indeed has a specific targeted (non generalised) effect on certain parts of the intestinal tract. I hope this helps.
