... ja, sofern es sich um einen österreichischen oder schweizer Text handelt - der von Nicole Büchel angegebene Link ist ein österreichischer und fast alle Fundstellen im Netz sind entweder österreichische oder schweizer Webseiten.
In Deutschland wäre Substitutionsvollmacht wohl eher Untervollmacht - damit lässt sich aber die Abkürzung SV nicht erklären.

Could be "[mündliche] Vollmacht und schriftliche Vollmacht erteilt", but I'm just guessing.

This is the entire sentence. It comes within the introduction of the parties. The context is a court pleading. After the Plaintiff is introduced, the identity of the lawyer follows, and the phrase in question comes below that. Clearly it relates to the lawyer.
