This is on a medical report from a Swiss hospital relating to a patient with a severe laceration injury.

Does it mean the decision on his inability to work is pending or that his inability to work is ongoing?

I think he was inpatient at the time it was written. It is on a letter written to the doctor of his home country to where he was returning.

Swiss German is a world unto itself but I would see it as "pending"

If we are talking about hospital INPATIENT treatment, then a patient is never fit to work while staying in hospital. In Germany at least, the patient's employer would receive a hospital certificate that is called something like "Bescheinigung über einen stationären Krankenhausaufenthalt".

I believe that a doctor's sick note (medical certificate of inability to work) is only issued after a patient has left hospital.

Mein Freund wurde holterdipolter ins Krankenhaus verfrachtet.
Ich habe mir dort mühsam eine Bescheinigung über die Aufnahme
erkämpft, jedoch keine AU-Bescheinigung erhalten. Man gab mir
die Auskunft, die Klinik dürfe eine solche Bescheinigung nicht
ausstellen.

Die Krankenhäuser stellen tatsächlich keine (gelben) Arbeitsunfähigkeitsbescheinigungen aus, es sei denn der Krankenhausarzt hat auch gleichzeitig eine Kassenarztpraxis. In so einem Falle ist es auch ratsam, sich an die Krankenkasse zu wenden. Wir erstellen anhand der Aufnahmeanzeige des Krankenhauses entsprechende Bescheinigungen für Arbeitgeber aus.

From a Swiss website I see that in Switzerland specialist hospitals specifically treating work-related accidents may issue inability to work certificates, so I conclude that all other hospitals may not.

In einer Notaufnahme werden nur Notfälle behandelt. In der Regel erfolgt dort keine gewöhnliche ambulante Betreuung. Arbeitsunfähigkeitsbescheinigungen werden nicht ausgestellt. In Kliniken mit unfallchirurgischer Abteilung ist häufig die Behandlung von Arbeitsunfällen (D-Arzt-Verfahren) der Notfallambulanz räumlich angegliedert, wobei von diesen nach Arbeitsunfällen auch Arbeitsunfähigkeitsbescheinigungen ausgestellt werden können.
