"Adnexen" are "adnexa": I'm not absolutely sure, but I think the adnexa (also referred to as annexa) are accessory organs connected to the uterus, e.g. Fallopian tubes etc. I would think that "ligation of the adnexa" would be sufficient for "Ligatur der Adnexverbindungen" in this case. HTH!
