Explanation:
From my German Routledge Dictionary:
Vollkaskoversicherungspolice = fully comprehensive insurance.

vollkaskoversichert sein = have fully comprehensive cover.

But Vollkosten = full cost. I would therefore suggest "Full Service Health Insurance".
