tertials = three 16-week rotations (Practical Year, Subinternship)
Explanation:
I would use "tertials" as is and add a translator's note, if necessary.

subinternship = 4th year medical student in North America.

internship = 18-month employment in a hospital, private practice or university institute. Culminating in Full Licensure to practice medicine

Three 16-week rotations in: Surgery, Internal Medicine and an elective of the student's choice. One or more of these tertials can be performed in a foreign university hospital or teaching hospital.

Only in Giessen can one of these tertials be divided into two 8-week periods: one 8-week period performed in a Medical School in the USA, the immediately following 8 week period in Giessen in the same discipline.
Practical Year culminates in the Third Clinical State Examination (oral).
